Minor Consent Form

A minor, named above, is applying to volunteer at The Baltimore Station. Part of the
volunteer process includes reviewing the Volunteer Handbook and filling out the forms
included in the back of the Volunteer Handbook. As parent/guardian of the above-referenced minor, I understand the purpose of these pre-volunteer procedures and
hereby provide my consent for the review of the Volunteer Handbook and filling out of
forms included in the Volunteer Handbook. I also allow my child to volunteer at The
Baltimore Station.
